Classification

•Antipsychotic


Action

•Unknown - a butyrophenone that probably exerts antipsychotic effects by blocking post synaptic dopamine receptors in the brain


Indications

•Psychotic disorders

•Chemical restraint of combative patients


Contraindications

•Hypersensitivity to the medication

•Patients with Parkinson’s, coma or CNS depression


Precautions

•Use cautiously in elderly and debilitated patients

•Patients with a history of seizure disorder, ECG abnormalities, severe CV disorders and glaucoma

•Patients on aniticoagulants, anticonvulsants, antiparkinsonians or lithium


Adverse Reactions

•Tachycardia, hypotension, hypertension

•Torsades de pointes, ECG changes

•Blurred vision

•Drowsiness, headache, insomnia, confusion

•Dry mouth, anorexia, nausea, vomiting

•Skin rash, urine retention


Interactions/Compatibility/Stability

•Anticholinergics, CNS depressants

•Lithium and alcohol use


Special Considerations

•Protect form light (slight yellowing of injection is common)

•Can be mixed in the same syringe as lorazepam or midazolam


Dosages

•Preparation: 5 mg/ml vial

•Adult: 2 - 5 mg IM/IV, every 5 - 10 minutes to a maximum of 30 mg

•Pediatric: N/A


Kinetics

Onset:         < 10 minutes

Peak:          10-20 minutes

Duration:    4-6 hours
